Responsible for managing and facilitating the transfer of patients from a hospital, skilled nursing facility and other health care providers to the care of home care. The coordinator is responsible for arranging uninterrupted continuing medical care of the patient, assisting in the organization of family resources for the effective care of the patient at home. The home care coordinators duties may be performed at the hospital or other health; Wisconsin RN or LPN license or WI Respiratory Therapist license or Social Worker license is required.
